Living with unpredictable IBS diarrhoea: Zara’s Story
Zara worked in corporate job which involved being visible and available to clients throughout the day, with little flexibility to step away if she needed to. For months she’d been living with unpredictable episodes of diarrhoea, loose stools, bloating and cramping that could start with almost no warning.
She’d already been through a full round of medical tests, all of which came back within normal range. This is a common and often frustrating experience for people I work with. You might be reassured that nothing serious is wrong but you still have constant digestive symptoms. Zara suspected her diet played a role but wasn’t sure how to work it out. She’d spent two years trying to follow the low FODMAP diet on her own, without much success.
The hidden cost of IBS symptoms
What made things particularly hard for Zara wasn’t just the physical discomfort. It was how much her symptoms were making her life smaller. She missed gym sessions she’d normally have enjoyed, not wanting to risk being caught out mid-workout. She sometimes had to phone in sick to work when her stomach felt too awful to manage a full day of client meetings.
By the time she came to me, Zara was leaning on anti-diarrhoea medication several times a week just to feel confident leaving the house. She’d also cut out an enormous list of foods trying to get ahead of her symptoms. Eggs, dairy, coffee, alcohol, spicy food, garlic, onions and fruit were all off the table.
Despite all of this restriction, she was still eating wheat freely, in the form of daily pasta and bread, because she’d come to see toast and pasta as safe. She desperately wanted to eat better and feel more like herself, but had no clear plan for how to get there. I see this quite a bit, that you’re doing their best with the information available, but without personalised guidance, you can end up with your diet evenmore restricted and no closer to feeling well.
Setting a goal: Feeling confident for her wedding and honeymoon
Zara wanted to understand her body better and feel more in control day to day, and she had a huge deadline in mind – She was getting married and going on honeymoon! She wanted to get to the wedding feeling confident and calm rather than anxious about what her bowels might do.

A personalised approach to IBS-D
With nine months to work with before the wedding, I started with a full food and symptom diary, tracking not just what Zara ate but when, and how she felt afterwards. This gave me a real picture of how things were at the time.
The diary showed she had limited food variety, and that wheat, a food many people don’t associate with IBS symptoms, appeared multiple times a day. Some of the exclusions she’d restricted on the basis they are typical IBS triggers rather than knowing they were an issue for her.
So rather than adding any more restriction, I worked with Zara to carefully expand her range of comfortable foods. I also worked with her to rebuild fibre and variety in a way that felt manageable, not overwhelming. Zara was nervous of vegetables and fruits and relied heavily on ready meals. Growing up she hadn’t had a lot of fresh foods, so we spent a bit of time working through her mindset, alongside practical changes to how she shopped, cooked and prepared meals.
Small, sustainable changes made the biggest difference.
A mid morning snack of fruit and a handful of nuts at work, aiming for at least two vegetables on her plate at each meal, and switching to frozen vegetables, which are cheaper, less wasteful for someone cooking alone, and a good source of fibre.

How things changed for Zara
At the start of our work together, Zara was relying on anti-diarrhoea medication several times a week, but would sometimes take too much and would then have constipation creeping in alongside her other symptoms. As we built up her soluble fibre intake gradually and consistently, Zara noticed her digestion felt steadier and more predictable, and, in consultation with her GP, she found she needed her medication less over time.
By the end of the three months, Zara told me her digestion felt calmer and more consistent than it had in years, and she no longer felt anxious about leaving the house without a plan for urgent dashing to the loo.
Life after the Gut Reset: Zara’s wedding and honeymoon
Zara went into her wedding countdown with far less background stress about her stomach on the day itself. She was excited to go on her long-haul flight for the honeymoon without the worrying of urgency and cramping.
She spoke about a real improvement in how she felt day to day, and valued having a nutritionist who was approachable and easy to talk to throughout. Her only regret, in her words, was not starting sooner.
